Blood use in elective surgical procedures in a “type A” base hospital

IntroductionBlood orders for surgical patients make up a considerable portion of total blood requests but the majority doesn’t lead to transfusion. Routine crossmatching and reserving all surgeries can be reduced by establishing Maximum Surgical Blood Ordering Schedule [MSBOS]. We have analyzed utilization in Base Hospital Puttalam formulated an MSBOS with this study. Material methodsA 2145 from gynaecology & obstetric units year 2019 was analyzed. Patients who underwent massive transfusions having pre-operative haemoglobin concentration less than 8.3g/dl or obtained transfusion were excluded considering correct preoperative anaemia. The crossmatch ratio [C: T], probability [T%] index [TI] calculated each procedure. procedures C: T >3:1 recommended group screen [G&S]. ResultsOf patients, 90% females. A 1565 requested 1521 crossmatched 2034 while only 193 transfused. Overall indices study population are CT 7.8:1, %T 10.4% TI 0.15. Indices unit 5:1, 15.2% 0.29. By using 1403 cross matches could been avoided. ConclusionsBy implementing 92% elective results on par previous studies done at tertiary health care centres.
